re ./pdf/out and./pdf/cache - seems as if its write only by root (and the pdf app needs to write to those and it runs as www-data)

chown those dirs to www-data and see if pdf works

wosrt case scenario is to chmod 777 -Rv ./pdf

let me know how you go

Just a little nitpicking here. www-data is assuming that your apache is running under that user which depends on the OS/distro you have.

Maybe someone has same problem. Then now is this pdf directory not under simpleinvoices root, but under include. So when You are getting cache directory errors, then look to /include/pdf

I spend right now 20 minutes to discover that :)
